Major Chemical Safety Incidents – A Review of Common Causes

This course will review incident investigations from The U.S. Chemical Safety and Hazard Investigation Board (CSB) with a focus on gathering data on common causes & failure modes and severity data, as well as any links between the facility’s EPA compliance history and major incidents.

Major chemical accidents carry risks to employees, the environment, the community, and to company reputations. As regulatory agencies and the public continue to focus on chemical management practices and Environmental and Social Governance (ESG) gains in importance, companies have a growing urgency to learn from and prevent these incidents. This course will review incident investigations from The U.S. Chemical Safety and Hazard Investigation Board (CSB) with a focus on gathering data on common causes & failure modes and severity data, as well as any links between the facility’s EPA compliance history and major incidents.

Learning Objectives:

After completing this course, participants should be able to:

  • Discuss an overview of several major industrial chemical incidents investigated by the CSB over the last 10 years within the US.
  • Identify commonalities between the incidents and understand basic patterns and trends within the investigation data, related to root causes and regulatory compliance history.
  • Explain how failures within a safety management system can be a key factor in high hazard facility incidents, and identify take-aways for our own risk and safety management practices.

Dana Garber, Senior Solution Strategist, Risk, VelocityEHS

Dana Garber, Senior Solutions Strategist at VelocityEHS, brings extensive knowledge of risk management, particularly in petrochemical hazards, to the VelocityEHS product team. His experience includes hazard identification, safety management systems, HAZOP studies, risk assessments, permitting processes, and facility siting. Prior to his current role, Dana spent ten years as a technical safety and risk consultant in the Oil & Gas industry, both on and offshore. Dana has received a Bachelor of Science Honors Degree in Mathematics from the University of York in the United Kingdom.
