Major chemical accidents carry risks to employees, the environment, the community, and to company reputations. As regulatory agencies and the public continue to focus on chemical management practices and Environmental and Social Governance (ESG) gains in importance, companies have a growing urgency to learn from and prevent these incidents. This course will review incident investigations from The U.S. Chemical Safety and Hazard Investigation Board (CSB) with a focus on gathering data on common causes & failure modes and severity data, as well as any links between the facility’s EPA compliance history and major incidents.

After completing this course, participants should be able to:
- Discuss an overview of several major industrial chemical incidents investigated by the CSB over the last 10 years within the US.
- Identify commonalities between the incidents and understand basic patterns and trends within the investigation data, related to root causes and regulatory compliance history.
- Explain how failures within a safety management system can be a key factor in high hazard facility incidents, and identify take-aways for our own risk and safety management practices.
